This On-Air Booking will not be scheduled until payment has been processed. Payment by check may delay the start date for this job. Once work has been scheduled any changes to the work order may result in additional fees. Due to the nature of production, once payment has been processed no refunds will be possible. No shows and cancellations will forfeit 100% of the booking fee. A 50% rescheduling fee may apply for any studio reservation changes made after this form is processed. Full and accurate On-Air Guest information is required at the time of processing. Inaccurate or incomplete guest information may delay your booking date. A direct personal email contact address is required for each guest. Each radio guest is subject to this fee. If booking on behalf of multiple guests, please adjust the quantity field accordingly when completing transaction and be sure to submit an On Air Booking Form for each guest.
